Information only, not an offer of coverage
Content on this site is general information about health coverage options. It is not an offer to sell insurance or a guarantee of coverage, eligibility or price. Coverage is available only where we are licensed and where a plan is offered.
Rates, savings and eligibility
- Prices and savings shown are examples. Your actual rate depends on factors such as age, household, location and underwriting approval, and may change at any time.
- Some plans require a health questionnaire and approval. Not everyone qualifies.
- Some plans shown are private health plans and are not ACA marketplace plans. They may limit or exclude coverage for certain conditions or services.
- Benefit summaries on this site are simplified. The official plan documents control your coverage, limitations and exclusions. Always review them before you enroll.
Consent to be contacted
When you submit our form and check the consent box, you agree that PPO Harlow and Taylor Harlow may contact you at the phone number and email you provided about health coverage, including by call, text or prerecorded message that may use automated technology, even if your number is on a do-not-call list. Consent is not a condition of purchase. You can withdraw consent at any time by replying STOP to a text, asking us on a call, or contacting us below.

You agree to provide accurate information and to use this site only for lawful purposes. Don't submit information about another person without their permission.
Third-party services and links
Booking uses Google Calendar, and this site may link to other websites. Those services have their own terms and privacy policies, and we are not responsible for them.
Trademarks
Cigna, Blue Cross Blue Shield, Aetna, UnitedHealthcare, Humana and other names are trademarks of their respective owners. Their use on this site describes networks available through certain plans and does not mean PPO Harlow is affiliated with or endorsed by these companies. PPO Harlow is not affiliated with any government agency or HealthCare.gov.
No warranties
This site is provided "as is." We work to keep information accurate and current, but we do not warrant that it is complete, error-free or always available.
Limitation of liability
To the fullest extent allowed by law, PPO Harlow and Taylor Harlow are not liable for indirect, incidental or consequential damages arising from your use of this site. Nothing in these terms limits duties we owe you as a licensed insurance professional under applicable law.
